Posterior perilunar dislocation of the carpus or retrolunar dislocation is a lesion where the capitatum is displaced behind the lunatum, and constitutes the most frequent perilunar fracture dislocation. It should be considered asa therapeutic emergency and the treatment should be absolutely surgical. The authors report a case of pure posterior perilunar dislocation of the carpus following an accident on the public highway. The patient underwent an urgent reduction followed by fixation with two scapholunar, and triquetrolunar pins by the posterior and medial approach, and immobilization for six weeks. After a 12months follow-up, the functional outcomes were satisfactory.
